২। আবেদনপত্রে প্রার্থীর নাম, পিতা ও মাতার নাম, স্থায়ী ও বর্তমান ঠিকানা (মােবাইল/ফোন
নম্বর উল্লেখসহ), জন্ম তারিখ, নিজ জেলা, শিক্ষাগত যােগ্যতা, জাতীয় পরিচয়পত্রের অথবা জন্ম নিবন্ধন
সনদের নম্বর, অভিজ্ঞতা উল্লেখ এবং তারিখ ও স্বাক্ষরসহ নিম্নলিখিত কাগজপত্রাদি সংযুক্ত করতে হবে।
(ক) সদ্য তােলা ৩ (তিন) কপি পাসপাের্ট সাইজের সত্যায়িত ছবি। (খ) জাতীয় পরিচয়পত্র/জন্ম নিবন্ধনের
সত্যায়িত ফটোকপি। (গ) শিক্ষাগত যােগ্যতা ও অভিজ্ঞতার সনদপত্রের সত্যায়িত ফটোকপি। (ঘ)

কাউন্সিলর/পৌরসভার মেয়র কর্তৃক প্রদত্ত নাগরিক সনদপত্র)। (চ) চেয়ারম্যান বিআরটিসি’ পরিবহন
ভবন ২১ রাজউক এভিনিউ, ঢাকা-১০০০ এর বরাবরে যে কোন বাণিজ্যিক ব্যাংক হতে ১৫০/- (একশত
পঞ্চাশ) টাকার ব্যাংক ড্রাফট/পে-অর্ডার।
(অফেরৎযােগ্য) সংযুক্ত করতে হবে। প্রার্থী মুক্তিযােদ্ধা/শহীদ মুক্তিযােদ্ধার পুত্র-কন্যা অথবা পুত্র-কন্যার
পুত্র-কন্যা হলে আবেদনে উল্লেখসহ ঐ মুক্তিযােদ্ধা/শহীদ মুক্তিযােদ্ধার সার্টিফিকেট, মুক্তিবার্তা/গেজেটের

কর্তৃক প্রদত্ত নাগরিকত্ব সাটিফিকেট আবেদনপত্রের সাথে সংযুক্ত করতে হবে। তাছাড়া, অন্যান্য কোটায়
আবেদন করলে আবেদনপত্রের সাথে প্রয়ােজনীয় প্রমাণাদিসহ উল্লেখ করতে হবে। (জ) নির্ধারিত
তারিখের পর প্রাপ্ত সকল আবেদনপত্র গ্রহণযােগ্য হবে না।
(ঝ) সকল চাহিত সনদপত্রের ফটোকপি প্রথম শ্রেণির (গেজেটেড/নন-গেডেটেড) কর্মকর্তা কর্তৃক
সত্যায়িত হতে হবে।
৩। আবেদনপত্র আগামী ১১/০৪/২০২১ তারিখের মধ্যে অফিস চলাকালীন নিম্নস্বাক্ষরকারীর দপ্তরে
সরাসরি/কুরিয়ার/ ডাকযােগে পৌছাতে হবে।
৪। শূন্যপদ পূরণ সংক্রান্ত বিষয়ে সরকারের নির্ধারিত কোটা ও অপরাপর বিধি-বিধান/আদেশ এবং
আনুষ্ঠানিকতা প্রতিপালন/অনুসরণ করা হবে।
৫। খামের উপর পদের নাম ও প্রার্থীর নিজ জেলার নাম উল্লেখ করতে হবে।
৬। লিখিত/মৌখিক পরীক্ষায় অংশগ্রহণের জন্য কোন প্রকার টিএ/ডিএ প্রদান করা হবে না।
৭। মৌখিক পরীক্ষায় অংশগ্রহণের সময় সকল সনদপত্রের মূল কপি প্রদর্শন করতে হবে।
৮। কর্তৃপক্ষ পদের সংখ্যা হ্রাসবৃদ্ধি বাতিল করার অধিকার সংরক্ষণ করেন। নিয়ােগ সংক্রান্ত বিষয়ে
নিয়ােগকারী কর্তৃপক্ষের সিদ্ধান্তই চূড়ান্ত বলে গণ্য হবে।
